PROSPECTIVE PATIENTS

Do you accept insurance?

We do not accept insurance. Our priority is providing patients with individualized care rather than letting insurance dictate what care is provided. However, you may use HSA funds to pay for all of our services. We can also provide you with a letter of medical necessity for you to submit to your insurance company for reimbursement.

What are your fees?

We believe that all patients deserve an equal right to receive care and therefore we have affordable pricing on all our services. We accept HSA, cash, check, and all major credit cards. If you have specific concerns regarding the cost of care, please contact us and one of our friendly staff will assist you.

Are you a medical doctor?

Jackie is a Certified Family Practice Nurse Practitioner and is able to diagnose, prescribe, and perform minor surgical procedures. We also have a certified health coach and licensed social worker on staff who have extensive knowledge and experience in health and life coaching. Our holistic approach to medicine can work in conjunction with standard medical care.

Do you prescribe medications?

Yes, we can prescribe medication if necessary. However, our primary focus is to help you establish healthy habits that fuel your body and remove the need for pharmaceuticals. In fact, helping people get off of their prescription medications is a priority for us.

Can you check my labs?

Yes. We offer an extensive bloodwork panel for only $225. This is performed at the clinic and results are usually available within 1-3 days.

CURRENT PATIENTS

I’m out of my vitamins and supplements. Now what?

You may order your supplements by clicking here. You may also stop by the clinic to pick up any supplements you are needing to avoid shipping costs.

How do I access the patient portal?

Go to https://flourishmedicalpatient.md-hq.com/ and enter your login information to access your online patient portal.

How do I set up my online patient portal?

You should receive an email with instructions on how to create your account. After you enter your information, you will need to return to your email and check the confirmation email. Once your account is confirmed you will be able to access the online, secure portal with your login.

What about follow-up labs?

We will check follow-up labs 4 weeks after treatment. This follow-up lab appointment will be scheduled during your visit at the clinic. If this was not previously scheduled, simply give us a call to schedule your lab draw. Be sure to avoid rigorous exercise and to not take thyroid medication for at least 5 hours prior to repeat lab testing. There is no additional cost for the routine follow-up lab panels.
